Meritain Health – Member Login
Meritain Health is the Third Party Administrator (TPA) for the Jefferson-Lewis School Employees' Healthcare Plan. Employees can access their healthcare benefits, claims information, and more by logging in to their Meritain account here: https://account.meritain.com/.
1095-C Form Information
In accordance with H.R. 3797 and H.R. 3801, printed and mailed copies of the 1095-C form will no longer be automatically distributed to district employees or retirees. This change is intended to reduce paperwork and administrative costs for school districts.
From now on, eligible employees must request their annual 1095-C forms using the following [Google Form].
Reminder: 1095-C forms are not required when filing annual tax returns
