Safe to School Calls

If your child is going to be absent, please call (315) 649-2417 ext. 108 and leave voice mail message. In the event that a message is not received, the school nurse will call to confirm that your child is not in school. Regardless of confirmation, a written excuse must be submitted for any day your child is absent.

Physicals

New York State requires a physical all new students and students in grades 7, 9, and 11. If your child is planning to play a sport, they will need a new physical each school year.

We are required to have a health update for each sport a student plays during the school year. Dr. Gianfagna, Lyme's school physician, provides physicals at school for no charge. You may choose to have your child's physical done by your own physician; in this case a physical form will be sent home for you.

Screenings

Your child will be screened for vision, hearing, height, weight, muscle balance, color vision and near vision as required by New York State. If your child has problems during the screening, you will receive a call from the school nurse and a referral letter will be sent home to be completed by your child's doctor.

New York State requires schools to track each student's Body Mass Index (BMI) and weight status category, as reported on the physical examination form. Each year, the New York State Department of Health (NYSDOH) will survey schools and collect this information (no identifying information is included). Parents who wish to exclude their child's data may contact the school nurse to receive a form to complete to remove their child from the survey data.
